IMMEDIATE REMOVAL !

The employee should be removed from all safety-sensitive and safety-related duties without delay.  This includes parking the truck/vehicle/machine if still on the road.

STEP 1

MAINTAIN VISUAL CONTROL !

If the offender is on-duty, you and another Supervisor should politly escort the employee to a private office to discuss continued employment options.

PRESENT VIOLATION TO OFFENDER

This is the time to present your test findings, and inform the employee of the specific Policy violation. 

Stay alert & safe during this meeting.

TERMINATION or LAST CHANCE

Stay consistant in accord with your company's culture.  If the offender really wants help, complete and follow all the terms outlined within your Company's Last Chance Agreement (LCA).

COMPLETE LAST  CHANCE AGREEMENT OR NOTICE OF TERMINATION

Take immedate action, without any time delay or waiting period.  Last Chance?  Complete & send it to FDTSI ASAP!

ARRANGE SAFE RIDE HOME

Provide safe transportation home for the offending employee.  Taxi, Uber, Lyft or Family Member.  The employee should not be permitted to drive themselves home.  The employee may still be DUI!
